What this means for your family

While many families appreciate the intimate, homelike setting and dedicated therapy team, recent reports indicate a decline in organizational oversight and safety. We strongly recommend that you conduct an unannounced visit to check for cleanliness and ask management specifically about their current protocol for medication administration and fall prevention.

Key Review Excerpts

The staff has been phenomenal in building relationships with him, the therapy department has tremendously helped him rehab post stroke, had open communication, and there is very frequent attention to his care.

Long-term resident's family · 2024★★★★★

The staff was very nice but this place is very unorganized!! Everyday my brother was in here I had to ask to get his shower, change his clothes, brush his teeth, get him ice, and water for his cup. Change his bed. Help him to the bathroom. It always smelled bad. Gnats all over.

Long-term resident's family · 2023★★☆☆☆

Since they took over it has been a nightmare. Not enough staff and very unorganized. My mother had two falls due to them not getting her psychotic medications, one of which caused a fracture in her neck.

This facility shows persistent safety and infrastructure problems across multiple surveys, with families having filed complaints leading to violations for inadequate abuse investigation response and infection control. The most recurring issues involve fire safety and emergency preparedness systems, electrical and power equipment, and dietary services. While all deficiencies show correction dates, the pattern of repeated violations in critical safety areas—including emergency power, fire barriers, and smoke detection systems—suggests ongoing maintenance and operational challenges that families should carefully consider.
