The investigation was initiated based on a complaint regarding the lack of hot water for residents. While the facility was found to have hot water on the date of inspection, interviews and record reviews indicated that the facility failed to ensure hot water or provide adequate accommodations when service issues occurred. The deficiencies cited relate to general maintenance, operational safety, and failure to maintain necessary bathing facilities.

The facility underwent a required comprehensive annual inspection. The inspection covered physical plant, food service, and care/supervision areas. Overall, the facility was found to be clean, in good repair, and operating in safe conditions, with no deficiencies cited.

The investigation was conducted following a complaint regarding the safeguarding of a resident's personal belongings. The allegation was substantiated because the facility could not provide evidence of a completed personal inventory for Resident #1. The primary deficiency cited relates to the failure to maintain and follow up on resident personal property inventories.
