The facility underwent a Case Management visit on November 17, 2025. The purpose of the visit was to obtain a resident file and secure a signature on an amended report. No deficiencies were cited during this visit.

The inspection revealed numerous deficiencies across multiple areas, including immediate health risks related to wound care, catheter management, and diabetic care (Type A). Several Type B deficiencies were also cited concerning staff training documentation, resident rights regarding mail, and facility governance. The facility was issued multiple civil penalties due to the severity and breadth of non-compliance found during the visit.

The inspection was an unannounced Case Management Annual Continuation visit. Multiple deficiencies were cited across several areas, including facility operations, incident reporting, staff training, dietary care, and technology access. All cited deficiencies were classified as Type B, indicating non-compliance that could become a risk if not corrected.

The unannounced Annual Required inspection identified multiple critical deficiencies related to resident safety and staff compliance. Specifically, the facility lacks proper fire clearance for bedridden residents, and there are multiple issues concerning staff background clearances and required documentation transfers. Immediate corrective action is required for these Type A violations to ensure resident safety.

The investigation was conducted following a complaint regarding the mishandling of resident medications. The allegation was found to be substantiated, leading to a citation for improper medication storage security. The facility must correct this by ensuring medications are stored in a safe, locked place inaccessible to unauthorized personnel.

The facility underwent a follow-up Prelicensing visit. The evaluator noted that the water temperatures in both bathrooms were tested and that pre-licensing deficiencies have been resolved, concluding the pre-licensing process. No deficiencies were cited during this inspection.
