This facility has concerning patterns with multiple complaint-triggered deficiencies, including families reporting issues with resident safety and abuse protection, with one recent abuse protection violation still lacking a correction plan. The most recurring problems involve resident safety hazards, medication management, and care planning issues. While most deficiencies show correction dates, the persistent safety violations across multiple surveys and ongoing complaints suggest systemic issues that may not be fully resolved.
