Key Review Excerpts

I’ve been to many LTC facilities and the staff at Evergreen House Health Center are top notch. From the receptionist that greets you knowing off the top of her head what room patients are in, to the aides feeding patients like their a family member, housekeeping talking in the halls to patients in Portuguese, and nurses that make this hospice nurse feel welcomed.

Hospice nurse · 2025★★★★★

The nurses are always great and caring towards my mother who has Lewy Body Dementia/Alzheimer’s, and Parkinson’s Disease. I also want to say that nurse Olivia, Eddy, Heather are very special people at the Evergreen Home and Nursing.

Half of Evergreen House's deficiencies resulted from family complaints, indicating ongoing resident safety and care concerns. The facility has repeatedly struggled with nutrition and food safety, care planning, and resident protection from abuse and neglect, with these issues persisting across multiple surveys from 2023 to 2025. While all deficiencies show correction dates, the pattern of repeated problems in critical care areas suggests systemic challenges that families should carefully consider.
