What this means for your family

While Greenwood Meadows is praised for its clean facility and effective rehab therapy, the frequency of reports regarding neglect and unprofessional behavior is concerning. If you consider this facility, we strongly recommend unannounced visits and asking for a clear plan on how they communicate falls or changes in medication to family members.

Key Review Excerpts

My father was here twice, the second time, he ended up with a bed sore that got so infected he was admitted to the hospital and was on antibiotics for weeks. I would never put another loved one in this facility.

Long-term resident's family · 2024☆☆☆☆

My grandmother is currently in the memory care unit. I visit every weekend to see her... All of went missing within a weeks time. I brought this up to the nursing staff in the unit, only to be told it’s a “memory care unit the patients can’t remember”

Memory care family member · 2024☆☆☆☆

My brother had to stay there for 5 months in 2023... My only complaint from a family perspective was he did have a couple of falls which I was not notified of. I found out later from him.

Greenwood Meadows shows concerning patterns with fire safety issues recurring across multiple surveys, including sprinkler system problems and emergency power deficiencies that remain uncorrected from the most recent December 2025 inspection. Families filed complaints in 2024 regarding abuse protection and incident response, though these were reportedly corrected. The facility has struggled with nutrition, resident care planning, and building safety compliance across all recent surveys.
