Type of inspection: Initial Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 10/23/2025 arrival time: 11:00am departure time: 12:00pm The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of residents present at the facility at the beginning of the inspection: There was 1 resident currently residing at the facility. The facility has occupancy for 8 residents. The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of interviews conducted with staff: 1 Observations by licensing inspector: Inspector completed a tour of the physical, including the building and grounds. Inspector took measurements in the 7 prospective resident rooms, observed the required furniture and observation and review of the First Aid kit. The facility is also equipped with an outdoor space for residents with a heated seating area for the cooler change in weather along with a resident garden. Additional Comments/Discussion: An exit meeting will be conducted to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
