What this means for your family

While the rehabilitation department receives some positive feedback, the overwhelming number of reports regarding neglect, hygiene issues, and understaffing makes this facility a high-risk choice. If you are considering this facility, we strongly advise conducting unannounced visits during weekends and evenings to observe the actual level of care provided, as these are the times families report the most significant lapses.

Generations at Applewood receives highly polarized feedback, with many families reporting severe neglect, chronic understaffing, and poor hygiene, including persistent odors of urine. While some reviewers praise specific staff members or the rehab department, the recurring reports of residents being left in soiled clothing, missing personal items, and experiencing delayed medical attention raise significant concerns for prospective families.

Key Review Excerpts

The front side is beautiful, staff seem to care, it's clean and modern. Thats where all the website pictures are taken. That OTHER SIDE...is a mess. The complete opposite. Dim, smells like urine, patients are not attended to.

Families have repeatedly filed complaints about this facility, triggering many of the 84 total deficiencies across 18 surveys. The most persistent problems involve accident prevention and safety hazards, pressure ulcer care, and medication management, with these issues recurring multiple times even after supposed corrections. While most deficiencies show correction dates, the pattern of repeated violations in the same care areas suggests ongoing systemic problems that merit careful consideration before choosing this facility.
