Key Review Excerpts

The nurse that I spoke to at this facility is a terrible, terrible person. She had an African/Nigerian accent just to narrow it down. I called up there to check on a relative that had been a long time resident, whom I hadn't heard from in a couple of years. When I asked how my relative was doing this 'nurse' responded with, 'The woman is dead. D E A D dead,' then hung up the phone.

Family member · 2024☆☆☆☆

I think the positive reviews from others have to be fake . This place smells like a combination of strong old urine and stale smoke . We brought my dad here for cardiac rehab . The worker just shows you a bed with zero greeting or instructions except to tell you they have no meds until the next day.

Rehab patient's family · 2024☆☆☆☆

Had I known that this place looked smelled I wouldn’t put a dog in there... The walls is peeling. The ceiling is coming down.the shower, had water bugs she was only in there for one night and the next night the hospital called she had 14 stitches.

This facility shows concerning patterns with families filing complaints about resident protection and safety supervision across multiple years. Issues with abuse prevention, accident hazards, and medication management recur throughout recent surveys. While all deficiencies report correction dates, the persistence of safety-related complaints and the facility's history of over 80 deficiencies across building safety systems suggest potential ongoing quality concerns that warrant careful evaluation during any visit.
