He had severe adema and had put himself on a wound vac so he needed to be in a medical setting. 3 days after he was transferred from the hospital to the nursing home TO RECOVER he died from a 'heart attack'. I had seen my dad the night before he passed and he said he would hit the call button and it would take sometimes hours for someone to come check on him.

A residents family member came to pick up family members for a visit and the family brought the resident’s wheelchair back in cause it was full of maggots and was so wet from urine cause CNA’s refuse to help residents in timely matter.
