Key Review Excerpts

My father was transferred there from the hospital. The room he was in looked like a storeroom. The paint was peeling off the wall, there was a huge tv turned upside down on a bed across from him, furniture sitting all over the place, closet was open and looked dirty.

Long-term resident's family · 2023☆☆☆☆

I am diabetic and ended up back at OSF because they gave me way too much insulin and I almost died. So yeah, I wouldn't trust them with a dog, let alone a member of your family.

Rehab patient · 2025☆☆☆☆

My Dad loved this place. He’d always call and tell me exciting stories about the activities they’d do and how he just made a new friend. The staff is amazing every time I came to visit my dad I was immediately treated with 5 star hospitality.

Fondulac has a concerning pattern of repeated safety violations and multiple family complaints over three years. The facility struggles most with accident prevention and safety supervision (cited 8 times), medication management (6 citations), and abuse/neglect reporting procedures (5 citations). While all deficiencies show correction dates, the recurring nature of safety issues—particularly accident hazards appearing across both routine inspections and complaint investigations—suggests ongoing challenges maintaining consistent resident protection standards.
