What this means for your family

While the facility has a strong rehab therapy team and some highly dedicated individual staff members, the pattern of neglect and communication issues is concerning. If you choose this facility, maintain a very active presence, visit at irregular hours, and verify that your loved one's specific care plan is being followed by nursing staff.

Key Review Excerpts

The therapy team at the facility was amazing. They worked with my mother and she progressed so much during her stay.

Rehab patient's family · 2025★★★★★

The staff members were more interested in visiting amongst themselves than helping us to get her settled. While waiting, my stepdad and I checked out the bathroom, which is supposed to be shared by FOUR people. When we saw that the toilet was splattered with feces, we rolled her right out of there and took her home.

Rehab patient's family · 2022☆☆☆☆

My mother has had over 80 pieces of clothing lost. I have found her pills on her bed and table 7 times. The person who gives her food will not wear a mask... Found a vile of blood in her bed.

1deficiencies
Well below state avg (3.9)

Families filed complaints in 2023 about care planning and resident assessment issues, which should be carefully evaluated during visits. This facility has recurring problems with fire safety systems, care planning, and infection control across multiple surveys from 2018-2023. While all deficiencies show correction dates, the pattern of repeated fire safety violations and the recent complaint-triggered findings about quality of care warrant thorough discussion with staff about current practices.
