The inspection was a Case Management visit conducted in conjunction with a complaint. The primary deficiency cited involved the improper storage of cleaning chemicals, specifically observing an unattended mixture of Clorox toilet bowl cleaner and Bleach in the Memory Care unit. This was classified as a Type A deficiency due to the immediate safety risk posed to residents.
