The facility failed to ensure that a medication aide (Staff A) had documentation of passing the required written medication aide examination within 60 days of being hired as a medication aide. Although the staff member completed the 15-hour training and clinical skills validation, there was no evidence of a passing exam score on file.

The facility failed to administer medications as ordered for one resident. Specifically, there was no documentation that nystatin powder was applied to a resident's groin for 9 separate opportunities across January and February 2022, despite active physician orders.

Specifically, there were multiple instances in January and February 2022 where the medication was not applied and not properly documented. Staff also incorrectly documented that medication was withheld per doctor's orders when it was actually due to medication unavailability or resident refusal.

The facility failed to ensure that 8 ounces of milk was served twice daily to residents as required by the daily menu. During a dinner meal observation, milk was not offered or served to 44 out of 45 residents present. Interviews with residents confirmed that milk was only served at breakfast and was not provided during the dinner meal.
