While the facility is noted for its cleanliness and small, home-like environment, the recent reports of poor communication and unauthorized patient transfers are significant red flags. Key Review Excerpts

The facility is very clean, neat and never had unpleasant odors. I always found the staff to be very helpful, knowledgeable and kind.

Long-term resident's family · 2025★★★★★

When he was sent back to the hospital, we received his bottled medications with 2 bottles missing. His seizure medication and his xanax. When we called to ask, they abruptly stated 'oh no we gave them back' .

Short-term resident's family · 2021☆☆☆☆

When I called them to get answers they hung up on me and when I tried calling back they would not pick up the phone and get back to me.

15deficiencies
Well above state avg (8.7)
1 complaint-triggered

Cra-Mar Meadows shows ongoing challenges with medication management appearing across multiple surveys, including a recent complaint-triggered deficiency for significant medication errors. The facility consistently struggles with staff training, infection control, and care planning, with issues persisting across all four surveys from 2023 to 2025. While all deficiencies show correction dates, the recurring patterns in critical care areas suggest systemic problems that families should carefully consider before visiting.
