Leaving my daughter stranded and worried about why this company promised they'd pick her up to do mental health services. Disappointed and sad, literally making her worse.”

Investigation involved allegations that a client threatened to use a bomb and had access to dangerous materials. Previous letter dated 06/25/2025 mentions that deficiencies for WAC 388-101-4150-1 and WAC 388-101-4150-2-a were corrected.
The provider failed to report aggressive behavior of a client for three of four incidents to the department as required, resulting in improper notification of acts of self-neglect and self-harm and placing other clients at risk.
