The facility failed to serve therapeutic diets as ordered for a resident who required a mechanical soft diet. Specifically, the resident was served non-ground meats and items like sausage links and chicken wraps that did not meet the prescribed texture requirements. This failure was observed during both lunch service on 08/13/24 and breakfast service on 08/14/24.

The facility failed to ensure that 8 ounces of milk were served three times daily to residents in the Special Care Unit (SCU). Review of menus showed no dairy products listed, and an inspection of the SCU refrigerator revealed no milk was available. Staff interviews confirmed that milk had not been regularly served to residents in the SCU for approximately one month.

The facility failed to maintain a heating system sufficient to keep resident rooms at 75 degrees F during winter conditions. Specifically, rooms E-3, E-9, D-1, D-7, F-2, and C-4 were not maintained at the required temperature. Additionally, thermostats were improperly located in linen closets near heat sources, making accurate temperature monitoring impossible.

The facility failed to immediately respond to an accident or incident for one sampled resident. Specifically, a resident was found on the floor, and there was no record of staff calling 911 at the time the resident was discovered, resulting in a 30-minute delay in calling emergency services.

The facility failed to ensure physician's orders were implemented for a resident with an order for weekly weights. Specifically, weights were not obtained on the required dates in January 2022, and staff were unable to identify who was responsible for obtaining them.

The facility failed to administer medications as ordered for two residents during a morning medication pass. This included errors regarding medications for GERD, allergies, pain, and asthma.
