This report details a complaint investigation conducted on August 28, 2024. Two deficiencies were cited, both classified as Type A, indicating immediate health risks. The primary concerns relate to inadequate staff training regarding resident hydration and the failure to ensure a specific resident remained properly hydrated, which was substantiated by hospital records.

The facility underwent an annual inspection conducted by the LPA. The inspection covered client records, personnel files, food service, and physical plant safety. The report explicitly states that there were zero deficiencies observed during the assessment.

The inspection identified one Type A deficiency related to having an unassociated, uncleared person in the food preparation area. The facility manager and LPA agreed on several corrective actions, including replacing fire extinguishers and implementing an infection control plan. The facility has committed to submitting a plan of correction by June 9, 2023.

The annual inspection focused on infection control measures. While the facility has adequate hand hygiene supplies and established protocols, two deficiencies were noted: the absence of visible Covid-19 postings and the lack of readily available PPE supplies. The licensee agreed to correct both issues before the end of the day.

The facility underwent an unannounced visit conducted as part of a case management review related to a complaint. The Licensing Program Analyst toured the facility and conducted resident interviews. No health and safety concerns were observed at the time of the visit.

The unannounced case management visit was conducted to review the facility's capacity increase request. The facility was found to be physically ready for the requested increase in capacity, and the LPA confirmed the approval of the new license following the visit.

The facility underwent a required annual inspection with an emphasis on infection control due to the COVID-19 pandemic. Inspectors observed several positive measures, including proper PPE use and established protocols for COVID-19 management. No deficiencies were cited per Title 22, Division 6, of the California Code of Regulations.
