What this means for your family

While the facility's dementia unit receives praise for compassionate care, the recurring reports of resident neglect, fall-related injuries, and poor facility maintenance are significant red flags. We strongly recommend that families conduct an unannounced visit to observe front-desk security and ask management specifically about their fall-prevention protocols and staffing ratios during night and weekend shifts.

Key Review Excerpts

Our 92 old mother in the final stages of dementia received excellent care to the very end. Everyone at every level was kind, caring, and saw to it that mom’s needs were met.

Memory care family member · 2023★★★★

On night 1, last night, he was left for 2 hours in his own urine and she had to drive from out of town to get him taken care of. This morning, he was served oatmeal with no milk or anything and a very black banana for breakfast.

Family member · 2025☆☆☆☆

No security at the front door. I have come to this facility 4 times and each time I was able to just walk in without having anyone at desk nor signing in. Scary that our most vulnerable people are a security risk.

48 complaint-triggered
$164,380 in fines

This facility has a concerning pattern of complaint-triggered deficiencies, with families filing 48 reports that led to violations in abuse protection, care quality, and safety. The most recurring issues involve protecting residents from abuse and neglect, providing appropriate treatment and care, and maintaining safe environments free from hazards. While all violations show correction dates, the persistent nature of abuse protection failures across multiple years and the high volume of family complaints suggest ongoing care concerns that warrant careful consideration before placement.
