Key Review Excerpts

My father frequently pressed the nurse's button to get assistance. More than half the time he would call me on his cell phone to tell me he was in pain and I had to call the nurse's station to get them to go into his room and assist him.

Rehab patient's family · 2021☆☆☆☆

The therapy staff was very patient with her and professional. I was worried from the negative reviews but pleasantly surprised. I had a few concerns, and was directed to a nurse name Bridget. She was extremely nice and patience, she address all of my concerns.

Rehab patient's family · 2024★★★★★

They left my mom all day for a week straight without a diaper change until they see me walk in the evening to visit her then the CNA walks in asking 'Do you need changing'.

This facility has a concerning pattern of family-filed complaints about abuse and neglect protection, with 33 complaint-triggered deficiencies out of 75 total violations. The most recurring issues involve protecting residents from abuse and neglect, accident prevention and safety supervision, and medication management errors. While all deficiencies show correction dates, the persistent nature of serious safety concerns—including repeated violations for abuse protection spanning from 2023 to 2025—suggests ongoing challenges with fundamental resident safety and care quality that families should carefully evaluate before considering placement.
