What this means for your family

While the facility is frequently praised for its physical therapy program and clean environment, recent reviews indicate a sharp decline in care quality and responsiveness. Families should be extremely cautious and are advised to ask specific questions about nurse-to-patient ratios and the facility's protocol for escalating medical emergencies to hospitals.

Agawam North Rehab and Nursing receives highly polarized feedback, with many reviewers praising the facility for excellent physical therapy and friendly, hardworking staff during rehab stays. However, there is a recurring and serious pattern of complaints regarding chronic understaffing, poor communication, and neglectful nursing care that has led to dangerous situations for residents. Families should be aware that while some report a clean and professional environment, others describe significant failures in medical oversight and responsiveness.

Questions for Your Tour

  • 1I noticed that your team is very active in responding to online feedback; how do you use that family input to improve the daily experience for residents?
  • 2Given the recent focus on medication management, could you walk me through your process for ensuring residents receive the correct dosages and timely follow-ups?
  • 3With 124 residents, how do you ensure that staff are consistently available to address non-emergency needs or requests throughout the day?
  • 4When a health concern arises, what is the specific chain of communication to ensure family members are kept informed and involved in the decision-making process?
  • 5What steps are being taken to improve the consistency of communication between the nursing team and families, especially during shift changes?
  • 6Since your therapy department is so highly regarded, how do you integrate those physical and occupational therapy goals into the resident's daily routine and social activities?

Key Review Excerpts

The PT and OT people were fantastic. Everyone was very nice and helpful during my stay.

Rehab patient · 2022★★★★★

The nursing staff, in particular is rude, uncaring and LAZY. I had to do things for my loved one, such as removing his heart monitor, charging it changing the electrodes and reattaching to his body!

Long-term resident's family · 2025☆☆☆☆

They refused to send my dad to the hospital when he needed it and I had to call 911 the second time myself. He ended up in the ICU and the facility never even contacted emergency services.

$86,130 in fines

This facility shows a concerning pattern of repeated deficiencies across multiple care areas, with one complaint filed by families that triggered a federal investigation in 2024. The most recurring problems involve resident rights violations, medication management issues, and infection control failures, with infection control cited in three separate surveys from 2022-2025. While the facility has corrected each identified deficiency, the persistent nature of problems across resident assessments, pharmacy services, and basic care standards suggests ongoing operational challenges that families should carefully consider.
