This report details a follow-up investigation conducted regarding allegations of staff misconduct, including yelling at and punishing a resident, and medication administration failures. All three allegations were ultimately determined to be unsubstantiated due to insufficient evidence.

COMPLAINT INVESTIGATION REPORT

The inspection was a Case Management visit conducted on August 14, 2025. While no immediate health or safety violations were observed during the visit, one deficiency was cited regarding fire clearance for a bedridden resident. This deficiency is classified as Type B, indicating a non-compliance issue that could become a risk if uncorrected.

The facility underwent a required annual inspection, and the Licensing Program Analyst conducted a thorough review of various operational areas. All inspected systems, including infection control, physical plant safety, and record-keeping, were found to be in compliance with regulations.
