What this means for your family

While many families report excellent experiences with specific staff members and the therapy department, the facility has a recurring pattern of communication failures and medication management issues. We strongly advise you to visit in person, observe the response time to call buttons, and ensure you have a direct line of communication with the unit manager rather than relying on the main office.

Key Review Excerpts

The CNA's and nurses are so attentive. The ones standing out are Sandy, Marie and Alexis. They always went the extra mile.

Rehab patient · 2025★★★★★

My Mom’s hearing aids were stolen from a locked nurses station cabinet in August 2025 and the administration won’t replace them. Also the brand new walker with a seat that Medicare paid for was delivered but never given to my mother.

Long-term resident's family · 2025☆☆☆☆

However today, at 11 am he had a severe fall. My mother went to go visit him around 5 pm and discovered this. They had not reached out to her, nor had they transported him to the ER. As they were “observing” him.

The facility has recurring issues across resident rights, care planning, quality of care, and nutrition/dietary services that persist across multiple surveys from 2022 through 2025.
