FACILITY EVALUATION REPORT

The facility underwent a Case Management - Annual Continuation visit on July 3, 2025. The Licensing Program Analyst reviewed staff and resident records, noting that all required documentation (Admission Agreement, Physician's Report, Appraisal Needs and Services Plan, Criminal Record Statements, and Health Screening) was present and up to date for all individuals reviewed. No deficiencies were cited during this inspection.

The facility underwent a Required 1-Year Annual inspection on June 16, 2025. The inspection covered resident apartments, common areas, and safety equipment, with positive findings noted in several areas. Overall, no deficiencies were cited during this visit.

The facility underwent an unannounced Required 1 Year visit and was found to have deficiencies in staff training and medication record-keeping. Specifically, multiple staff members lacked current first aid certifications, and records for centrally stored medications were incomplete for several residents. Immediate corrective action is required for both deficiencies.

The facility underwent an unannounced Case Management visit following a report of suspected abuse. The Licensing Program Analyst interviewed multiple staff members and residents, and reviewed various records. No deficiencies were cited at this time per California Code of Regulations Title 22.

The facility underwent an unannounced Required 1 Year visit. The inspector noted positive observations regarding the visitor screening area, bathroom supplies, PPE stock, and food supplies. No deficiencies were cited during this inspection.
