The facility lacked a physician’s order, PT/OT evaluation, manufacturer’s guide, staff education, and service plan inclusion for a resident's bedside assistive device with a support bar.

The ice scoop container was dirty with visible debris, and ice scoops were observed touching the soiled area.

Multiple perishable food items in the commercial kitchen’s refrigerator and freezer were observed without proper sealing or labels identifying when the packing was opened or when the items were prepared.

Two containers of dry goods (sugar and flour) had plastic scoops located inside the contents; utensils were not being sanitized after each use and were permanently housed inside the containers.
