Medication administration records (MARs) showed PRN medications (Acetaminophen, Lorazepam, Hyoscyamine) were administered for reasons inconsistent with physician orders or lacked a documented reason for administration.

Failure to provide reasonable protective measures for a resident using a 'Halo Ring' bedside device, including lack of physician orders, lack of manufacturer guidelines, and lack of staff training/monitoring plans.
