The facility failed to maintain an organized program for resident protection regarding bedside mobility aids. Specifically, service plans for several residents lacked instructions for the use, care, and maintenance of devices (like halo rings and U-shaped mattress devices), and there were inconsistencies between physician orders, the facility's SOP, and actual device use.

Lack of evidence for initial tuberculosis (TB) screening on record for an individual admitted to the residence within 12 months before admission.

The facility was found to have inadequate staffing levels. Specifically, on 5/18/2025, only two staff members were on duty for residents requiring two-person assistance and/or a Hoyer lift. Additionally, the schedule showed instances where no staff were assigned to memory care on certain dates in late May/early June 2025.

Lack of adequate action taken to protect Resident A following medication errors.

Misadministration of medication for Resident A, including a double dose of Propafenone and multiple instances where Metoprolol was administered despite orders to hold the medication.
