Key Review Excerpts

My mom was in there for 16 days we complained every day her food was cold ice cold when she finally got it. We would visit her for hours and on one would come to see if she needed anything I would always have to go get her ice water when I got there the cup was always empty.

Long-term resident's family · 2026☆☆☆☆

This is a very good place they have a very good staff they care about people they have a very good Physical Therapy Program I give them 10 Stars... I can walk again I love you all

Rehab patient · 2024★★★★★

While some of the floor staff have been wonderful, the business manager and administrator have left me feeling like this is a very dishonest place. All they seem to care about has been warehousing my mother in long term care, when she was admitted for rehabilitation with a goal of coming back home.

Van Buren Healthcare shows recurring issues across multiple care areas, with families filing complaints about safety hazards and daily care assistance. The facility has persistent problems with nutrition and dietary services, quality of care, and fire safety systems appearing across all three surveys from 2022-2024. While all deficiencies show correction dates, the pattern of repeated violations in essential areas like meal service, resident assessments, and safety systems suggests ongoing operational challenges that families should carefully consider.
