The facility failed to ensure proper referral and follow-up for a resident's health needs. Specifically, staff failed to notify the primary care provider regarding a fall that resulted in arm pain and bruising, and failed to notify the physician and wound care provider about redness, swelling, and drainage on a resident's leg wound.

The facility failed to meet the health care needs of a resident by failing to notify the physician of the resident's refusal to wear prescribed compression stockings for leg swelling. Additionally, staff failed to document these refusals on the Medication Administration Record (MAR) and did not notify the primary care provider of the non-compliance.

Observations included sticky substances on dining room and kitchen surfaces, dust/dirt build-up on baseboards and blinds, and mildew on the shower curtain. Additionally, loose laminate flooring in the kitchen, hallway, and bedroom entryway created trip hazards for residents.
