What this means for your family

While the facility is physically modern and offers effective therapy, the recurring reports of understaffing and poor response times are significant red flags. We strongly recommend visiting during a weekend or evening shift to observe actual staff-to-resident ratios and asking management specifically how they handle call light response times and medication management.

Superior Health & Rehab presents a stark contrast in experiences, with some families praising the beautiful facility and compassionate staff, while others report severe neglect and unprofessional management. Common criticisms include significant understaffing, slow response times to call lights, and poor communication regarding medical care and discharge planning. Families should be aware of these polarized reports and conduct thorough, in-person visits to assess current staffing levels and care quality.

Key Review Excerpts

The facility won’t staff an appropriate amount of CNAs/ Nurses to ensure that every resident gets proper care. Your family member will be lucky to be cleaned and changed twice in a 12 hour shift due to lack of staff.

My father is 91 and really struggled with being in rehab, but the amazing staff made his progress and recovery a wonderful experience! We were in rehab for about a month and we feel so blessed that these awesome people were there to support and encourage us!

Rehab patient's family · 2022☆☆☆☆

I walked through the halls and smell urine from residents' diapers not being changed. It's very sad. SO if you love your family do not bring them here.

Both RN and total nursing hours are below national benchmarks. This can mean less clinical attention per resident, so ask about their staffing plan.

2deficiencies
Well below state avg (4.3)

Superior Health & Rehab has 18 deficiencies across three surveys, with recurring issues in fire safety systems, care planning, and food service that persist across multiple inspections. Fire safety problems including alarms, smoke detection, and exit doors have appeared in all three surveys, while care planning deficiencies have occurred twice. All violations have been corrected by the facility, but the pattern of repeated fire safety citations suggests ongoing challenges in maintaining these critical safety systems.
