What this means for your family

This facility is highly regarded for its physical therapy and clean, private-room environment, making it a strong candidate for post-surgical recovery. However, given reports of inconsistent response times and hygiene lapses, we strongly recommend that families conduct frequent, unannounced visits to monitor care levels and ensure communication protocols are being followed.

Key Review Excerpts

The Physical Therapy staff are top notch.. Nate and Tyler and Kim are excellent ... all very professional and knowledgable with great "bedside manner"

Rehab patient · 2023★★★★★

My mother was taken to Stonehenge after her hospital stay. She has memory challenges and so it was difficult for her to be anywhere but home. The staff was wonderful and did everything possible to make her stay as comfortable as it can be while away from home.

Memory care family member · 2023★★★★★

The best thing about this place is there are PRIVATE ROOMS. Right there was a big determining factor of coming here. The place is clean and well-staffed.

2deficiencies
Well below state avg (9.1)

Stonehenge of South Jordan has 16 federal deficiencies across three surveys, with all violations reported as corrected. The most recurring issues involve fire safety systems (smoke detectors, sprinkler maintenance, emergency lighting) and infection control programs. Additional concerns included medication management, respiratory care, and food safety. While the facility has addressed each deficiency when cited, the pattern of fire safety violations appearing in multiple surveys suggests ongoing maintenance challenges families should inquire about.
