What this means for your family

While the facility is physically attractive and features a strong therapy department, the recurring reports of severe neglect, unresponsiveness to call lights, and medication errors are deeply concerning. If you consider this facility, you must conduct unannounced visits during weekends and evenings to verify staffing levels and observe the actual quality of care provided to residents.

Key Review Excerpts

Within a few hours of being in their care, and after being told multiple times my grandmother was a fall risk patient, she fell and broke her wrist and shoulder and had a gash on her head.

Memory care family member · 2019☆☆☆☆

The physical and occupational therapy is OUTSTANDING!! 20 ⭐️⭐️⭐️. If you take meds do NOT go here They gave way too much blood thinner and then didn’t know how to handle the emergency of her non stop bleeding.

Long-term resident's family · 2022★★☆☆☆

I’m very thankful For Stacey MA Monday through Friday on the 3 to 11 shift. I know when I come in in the evening and when I leave in the evening that my loved one gets the most amazing care when she has Stacey has an aide.

9 complaint-triggered

This facility shows significant concerns with families filing 9 complaint-triggered deficiencies, including serious issues with preventing abuse and neglect that prompted family reports in 2025. The most recurring problems involve medication management (appearing in 6 deficiencies across multiple surveys), resident protection from abuse and financial exploitation, and infection control. While all deficiencies show correction dates, the pattern of repeated medication issues and the recent serious complaints about abuse prevention and respiratory care suggest ongoing operational challenges families should carefully evaluate.
