What this means for your family

While the rehabilitation therapy team is frequently cited as a major strength, the facility has a recurring pattern of serious neglect and communication issues. Families should exercise extreme caution, conduct unannounced visits, and verify that the facility can meet specific medical needs before admission.

Emerald Care Center Midwest presents a highly polarized experience for families, with many reviewers praising the rehabilitation therapy team while others report severe neglect and communication failures. While some families describe a warm, family-like environment, numerous reports of poor hygiene, unaddressed medical needs, and inconsistent staffing levels raise significant concerns about the quality of care. Families should be aware that experiences appear to vary drastically depending on the specific shift and individual staff members involved.

Questions for Your Tour

  • 1I noticed that many families appreciate the active social calendar here; could you walk me through what a typical day of engagement looks like for a new resident?
  • 2Given that some families have expressed concerns regarding hygiene and bathing routines, what is your facility's specific process for ensuring residents receive consistent personal care and assistance?
  • 3I see that your team is very active in responding to feedback online; how do you maintain that same level of responsiveness when communicating directly with families about their loved one's daily status?
  • 4With the current staffing levels, how do you ensure that medication management is handled accurately and that residents receive their doses on time, every time?
  • 5Since physical and occupational therapy are highlighted as strengths here, how do you integrate those services into a resident's daily routine to ensure they stay active and mobile?
  • 6What protocols do you have in place to ensure that residents are receiving adequate nutritional support and that their dietary preferences are being met?

Key Review Excerpts

My husband had a traumatic brain injury and when looking for a skilled nursing facility I toured this one, I was impressed with the staff there. It is outdated looking on the outside but the faculty are caring and kind and take good care of the patients there.

Long-term resident's family · 2017★★★★★

My dad moved into Emerald Care Center in January 2022. I am impressed with the staff - from the director, patient advocate, maintenance, PT/OT, and especially the nurses and aides. They are kind and responsive. They talk to my Dad like he's an adult, not a child.

Long-term resident's family · 2022★★★★★

My mom was just moved from this nightmare. I would not recommend this place for anyone. The care is awful!!! Some of the aides are very rude and no compassion at all. My mom left with bruises all over her arms and not doing any better than she was when she came in.
