This report details the investigation of a complaint alleging that staff did not provide proper care to a resident. The investigation found that the allegation was unsubstantiated based on interviews, observations, and record reviews. No deficiencies were cited in this report.

The facility underwent a Case Management visit on October 3, 2025, regarding a Change in Administrator. The Licensing Program Analyst collected several documents and requested numerous updates and reports from the facility management. No deficiencies were cited during this visit.

The unannounced Annual Required inspection found the facility generally clean with appropriate supplies and services in place. However, one Type B deficiency was noted regarding unsecured light cleaning supplies in the assisted living area. The facility was otherwise found to be in compliance with most observed standards.

This report is a Complaint Investigation conducted on 01/28/2025. The investigation addressed four allegations concerning resident care, social interaction, dietary needs, and incontinence care. All allegations were found to be unsubstantiated by the licensing evaluator.

The facility was inspected on November 20, 2024, following an unannounced health and safety check related to an incident report from November 5, 2024. The Licensing Program Analyst found that the facility reported the incident in a timely manner and submitted appropriate documentation to all required agencies. No deficiencies were cited during this visit.

The facility underwent an Annual Inspection on May 31, 2024. The Licensing Program Analyst reviewed staff files and interviewed personnel. A review of centrally stored medications was also conducted, and no deficiencies were cited during the visit.

The facility underwent an Annual Continuation inspection on May 29, 2024. The Licensing Program Analyst reviewed resident and staff files. No deficiencies were cited during this visit, and the annual inspection will be completed at a later date.
