The facility failed to ensure that one of four sampled staff members was tested for Tuberculosis (TB) disease upon hire. Specifically, there was no documentation of a TB skin test for Staff C within the past 12 months or following the facility's license effective date.
The facility failed to access the Health Care Personnel Registry (HCPR) before hiring to ensure no substantiated findings for two of four sampled staff members. This lack of screening for housekeeping and medication aide staff meant the facility could not verify the safety of the residents.
