Key Review Excerpts

The facility is very basic and older, but clean. Megan, the Nurse Practitioner and the nursing staff and PT team were very caring, gentle, positive, friendly and professional, always going out of their way to make sure my father was well taken care.

Rehab patient's family · 2026★★★★★

I was there from Dec 2 - 25th 2025 with 2 broken legs a wrist and collar bone and had therapy there with Jordan and Lee and they were great I wasn't able to walk or move my right arm and Jordan did a great job getting my legs working and Lee with my arm.

Rehab patient · 2026★★★★★

My mother came into this facility not able to walk at all and she left with a big smile and is enough walking. I cannot reiterate enough how grateful me, and my family are for the staff's performance for making this possible.

7 complaint-triggered

This facility shows concerning patterns with families filing 7 complaints that triggered federal investigations, revealing issues with abuse/neglect reporting, resident safety, and care planning. The most recurring problems involve resident rights violations, infection control deficiencies, and quality of care issues. While the facility has corrected each identified deficiency, the repeated nature of complaints—particularly two abuse reporting violations within a month in late 2024—suggests ongoing challenges with maintaining consistent care standards.
