Key Review Excerpts

The moment you step onto any patient floor, you’re met with a strong, persistent odor of urine and feces. This isn’t an isolated incident - it’s a consistent issue that suggests a lack of routine cleaning and timely patient care.

Family member · 2026☆☆☆☆

I was here every day at the same time to visit family and everyday he is still in bed at 12-1230pm not dressed or washed up... and all the staff is sitting at the end of the hall chit chatting instead of working.

Family member · 2023☆☆☆☆

The staff on Forest Drive is an excellent very thorough group of people. It may be in an older building than the location in Blythewood but It was a great physical rehab experience

The facility has recurring problems with medication management, resident rights, and infection control that persist across multiple surveys from 2022 to 2025.
