Key Review Excerpts

My dad was placed here in November 2024, and he passed mid January 2025. When he was checked in here, he was in good health minus his Parkinson's. Within a month, he developed fast growing pressure ulcers all over his legs and butt that were a result of him not being proper changed and treated enough.

This has been a great place for my short term rehab! Staff has been so nice and sweet to me. Residents are all good people and easy to make friends with. I enjoyed the activity program and playing bingo.

This facility shows concerning patterns with frequent complaints from families, resulting in 43 complaint-triggered deficiencies out of 112 total violations. The most recurring issues involve accident prevention and safety hazards, resident abuse and neglect protections, and medication management problems. While the facility corrects violations when cited, the persistent complaints about safety hazards (appearing in multiple recent surveys through 2025) and repeated abuse protection failures suggest ongoing care quality concerns that families should carefully evaluate during visits.
