What this means for your family

While the facility's physical therapy department is highly effective for recovery, the recurring reports of neglect and poor communication are serious red flags. If you choose this facility, ensure you have a consistent presence to advocate for your loved one, and specifically inquire about their protocols for responding to patient call bells and staff-to-patient ratios on weekends.

Key Review Excerpts

The rehab staff pushed me in a positive way and now after 2 1/2 months I can now stand and walk.

Rehab patient · 2023★★★★

I NEVER FEEL ANXIOUS ABOUT LEAVING MOM. THE STAFF IS PLEASANT AND PROFESSIONAL. THEY OFTEN GO ABOVE AND BEYOND TO HELP HER FEEL SAFE AND CONTENT. WEEKENDS ARE A BIT SHORT STAFFED BUT I HAVE NO SAFETY CONCERNS.

Long-term resident's family · 2023★★★★

My 94yr old uncle fell off his bed and it took an HOUR before someone came to help. Someone picked him up threw him on his bed and did not check to see if anything was broken!

This facility shows a concerning pattern of recurring deficiencies across multiple surveys, particularly in care planning, resident rights, and daily care assistance. Issues with medication management, respiratory care, and infection control have persisted across surveys from 2023 to 2025, suggesting ongoing challenges in core care areas. While all deficiencies show correction dates, the repetitive nature across four surveys indicates potential systemic issues that families should discuss directly with facility leadership.
