What this means for your family

While the facility has received praise for its recent management changes and clean environment, the recent reports of inconsistent hygiene and serious clinical oversights are concerning. We strongly recommend that you conduct an unannounced visit to check the cleanliness of resident rooms and ask specifically about the ratio of permanent staff versus temporary agency staff.

Google Reviews

Google Reviews

The Pearl of St. Charles receives highly polarized feedback, with a significant number of recent five-star reviews praising the facility's leadership, cleanliness, and staff dedication. However, these positive reports contrast sharply with serious allegations from other families regarding neglect, poor hygiene, medication errors, and unprofessional communication. Families considering this facility should be aware of the starkly different experiences reported by residents and their loved ones.

Key Review Excerpts

My first impression of The Pearl (formerly River Crossing) is that they are one of the cleanest facilities I have visited. My father was a resident there for a short time and was given great attention and care by staff members.

Long-term resident's family · 2024★★★★★

The staff is primarily hired through a temp agency, meaning many of the nurses are not licensed, certified or properly trained. If your loved one has a feeding tube, expect severe neglect—they will not properly care for it or ensure the patient is fed.

Family member · 2025☆☆☆☆

The administrator, Sandra is highly involved and is genuinely concerned about the residents and their needs. Her leadership is a primarily reason why I strongly recommend The Pearl.

Families have filed 26 complaints triggering federal inspections, with recurring issues in medication management, accident prevention, and resident care standards. The facility shows persistent problems with medication errors appearing across multiple surveys from 2023-2026, and accident hazard concerns spanning from 2023-2024. While all violations are marked as corrected by the facility, the pattern of repeated complaints and similar deficiencies suggests ongoing care quality challenges families should carefully evaluate.
