The facility was inspected unannounced for a case management review concerning two prior incident reports. The Licensing Program Analyst reviewed the files and conducted interviews with the administrator. No citations were issued per Title 22 Regulations.
The facility was visited unannounced for a case management review. The Licensing Program Analysts met with the Administrator and reviewed necessary documentation. No citations were issued per Title 22 Regulations.
The facility underwent a Required-1 Year inspection on April 23, 2025. The Licensing Program Analyst toured the facility and reviewed resident and staff files. No immediate health, safety, or personal rights violations were observed during the inspection.
This report details the findings of a Complaint Investigation conducted on July 16, 2024. The allegation regarding staff failing to properly address a scabies outbreak was reviewed and subsequently found to be UNFOUNDED. No deficiencies were cited in this report.
