The facility underwent an unannounced case management visit to follow up on a reported resident fall. The Licensing Program Analyst conducted interviews with staff, the licensee, and the resident, and performed a file review. No citations were issued per Title 22 Regulations.

The facility underwent a Required-1 Year inspection on June 6, 2025. The Licensing Program Analyst conducted a thorough tour and review of records, finding no immediate health, safety, or personal rights violations. The facility was found to be in compliance with all observed standards.

The facility underwent a Case Management health and safety check following reported flooding. The licensee reported that affected resident rooms are drying out and residents currently have access to their rooms. The facility awaits final inspection clearance from the insurance company before completing necessary repairs.

The inspection found deficiencies in both personnel and resident record keeping. Specifically, there were issues with maintaining required employee documentation, including TB tests and first aid records, which present potential health and safety risks. Additionally, resident records were found to have outdated care plans and missing personal rights documentation.

The facility underwent a 1-Year Required Annual Inspection conducted unannounced. The LPA observed the facility to be clean, in good repair, and odor-free, noting proper safety equipment and adequate food/sharps storage. No deficiencies were cited as a result of today’s inspection.

The investigation report details two separate complaint investigations conducted on 06/16/2022. The first allegation regarding dispensing the wrong medication was found to be substantiated, citing a Type A deficiency related to medication administration. The second allegation was found to be unfounded.
