The workers (besides a VERY select few) do not care if these people live or die. The administrator, Patricia is EXTREMELY neglectful. I would put my dog in this place. They have mice and bugs! They refuse to call an exterminator. They do not serve round meals for these poor residents. A salad and apple sauce is not a round meal. I have found multiple pills on the floor that do not belong to my grandmother.

Specifically, staff administered incorrect insulin doses based on blood glucose readings, including an instance where 5 units were administered for a blood glucose of 220 when the order required 10 units for readings over 200. The medication administration record (MAR) showed a failure to monitor and follow the updated sliding scale instructions for several months.
