While the staff member had performed a check for a previous employer, there was no documentation of a completed background check on file for their employment at this facility.

The facility failed to ensure that criminal background checks were completed upon hire for two of three sampled staff members in accordance with state regulations. For one staff member, the background check on file was dated eight months prior to their date of hire.

The facility failed to maintain a clean and orderly environment free from hazards, as evidenced by an ant infestation in the kitchen area. Observations showed a moderate amount of ants on the counter near food items, and a previous inspection had also noted vermin control issues.

The facility failed to ensure all staff members were cleared through the North Carolina Health Care Personnel Registry. Specifically, a review of a newly hired Medication Aide/Supervisor revealed that no registry check was completed prior to the individual's hire date.
