What this means for your family

This facility's rehabilitation and therapy team is highly regarded and often cited as a reason for successful recovery. However, the recurring reports of neglect, slow response times, and poor communication are significant red flags. If you choose this facility, we strongly recommend frequent, unannounced visits to monitor your loved one's care and hygiene.

Key Review Excerpts

My family member had a great experience here. Great activities and rehabilitation team. Got so strong he could live by himself again!

Long-term resident's family · 2024★★★★★

The nursing staff and the technicians were excellent, but that is the only good I can say about the facility. The hallway upstairs constantly smells like human waste, my mother spent hours in soiled diapers every day, she would press the call button and nobody would come.

Rehab patient's family · 2025☆☆☆☆

The therapy team are great, clean facility, some of the nurses are so kind and compassionate and some of the techs are great. The A.D.O.N. is one of the kindest ,most caring ladies I ever met.

12deficiencies
Well above state avg (5.9)

This facility shows recurring deficiencies across multiple surveys, with persistent issues in care planning and assessment, fire safety systems, and building maintenance appearing in all three inspections from 2018 to 2023. While all violations have been corrected according to federal records, the pattern of repeated deficiencies in core areas like resident assessment, sprinkler system maintenance, and electrical safety suggests ongoing operational challenges that families should discuss during visits.
