The facility failed to provide adequate supervision for a resident identified as having wandering and exit-seeking behaviors, resulting in the resident eloping from the facility on 1/21/2024. Despite the resident's known history of wandering and disorientation, staff did not prevent the resident from exiting the building and walking approximately 211 feet away.
