The facility failed to maintain hot water temperatures at a maximum of 116°F, as evidenced by 10 out of 10 sinks in resident and common bathrooms exceeding this limit. Specific observations showed temperatures as high as 123.8°F. Additionally, a family member reported that the water in a resident's bathroom could become dangerously hot to the touch.

The facility failed to ensure that medication administration competency evaluations, consisting of a written examination and clinical skills evaluation, were properly conducted and documented according to state requirements.

The facility failed to ensure that 2 of 3 sampled staff members who administered medications had completed the required Medication Clinical Skills Competency Validation. Specifically, for Staff A, while training and exam results were verified, there was no documentation of the required clinical skills validation.

The facility failed to provide adequate supervision for two residents in the Special Care Unit, including a resident with a history of elopement and a hospice resident with a high fall risk. Specifically, the facility did not increase supervision or assign a sitter following a significant fall and subsequent injuries to a resident.

The facility failed to ensure that one medication aide and two personal care aides had been competency validated via return demonstration for licensed health professional support tasks. Specifically, there was no documentation of competency validation for tasks including applying compression stockings, obtaining fingerstick blood sugar checks, and assisting with resident transfers.

The facility failed to maintain hot water temperatures at a minimum of 100 degrees F and a maximum of 116 degrees F. Observations revealed several resident room sinks and community restroom sinks were either too hot (up to 132 degrees F) or too cold (as low as 88 degrees F).
