What this means for your family

The Leonard Florence Center offers a modern, home-like environment with highly-rated physical therapy, which many families find beneficial for recovery. However, there is a concerning pattern of reports regarding nursing neglect and poor communication. We strongly advise families to conduct frequent, unannounced visits and to establish a clear, documented communication plan with nursing leadership upon admission.

Key Review Excerpts

I love how home-like the facility is. The unit is bright and the staff always greeted us with a smile when we visited. This is NOT a nursing home. This was a home away from home where my mom got the care she needed to get back to her home safely.

Rehab patient's family · 2024★★★★★

The food is great and you have your own room and every floor has only 10 people and your own cook so you can eat at a dining room table or your own room.

Rehab patient · 2024★★★★★

My 96 year old mother was left in her chair for hours, developing a bed sore. She was supposed to receive nutritional supplements, I had to ask several times. Communication is terrible, social workers are temps and not really helpful.
