Medication log errors for Resident A: Staff failed to initial the medication log for Ammonium Lactate 12% Lotion (03/28/2022 and 03/29/2022) and Verapamil 40MG (10/30/2022 and 10/31/2022).
Bedroom #2 did not have a mirror for grooming.
