Key Review Excerpts

When I first arrived at Lakeshore Village, I was unable to stand. I am now able to walk with a walker. Their physical and occupational therapy are wonderful.

Rehab patient · 2025★★★★★

My father in law was placed here by the VA. He is a dementia patient. They have been told by his VA doctors and others he is not permitted to leave. They have let him leave many times which has placed him in bodily harm, danger of wondering off.

Memory care family member · 2023☆☆☆☆

My dad 87 years old with dimentia and PTSD was taken from providence Hospital to Lakeshore for physical therapy for a month and they had him sign papers and stole his Social Security check and had them direct deposit to their account!!

$135,984 in fines

This facility has a concerning pattern with 32 of 54 deficiencies triggered by family and resident complaints, indicating ongoing care issues that prompted external reports. The most recurring problems involve quality of care and safety (particularly accident hazards appearing multiple times), medication management, and nutrition services including food safety. While all identified deficiencies show correction dates, the high volume of complaint-driven citations suggests systemic challenges in maintaining consistent care standards, warranting careful consideration and thorough questioning during any visit.
